Job Details

Provides clinical, clerical, and care coordination support to providers and members of the interdisciplinary patient care team by performing a variety of back-office, patient-centered, and coordination-related activities. Plays a key role in supporting continuity of care by assisting with care planning, tracking patient needs, facilitating communication between providers, and coordinating services across the care continuum. Helps ensure smooth patient flow and efficient workflows within the department while supporting the implementation of the medical plan of care. Serves as a consistent point of contact for patients and families, assisting with follow-up, education reinforcement, referrals, and care transitions to promote understanding, adherence, and timely access to services.
