Suncrest Hospice - - Responsibilities: Provide resident care to assigned residents in accordance with resident assessments and care plans; Coordinate proper types of care with licensed care staff; Follow all Company nursing policies and procedures and safety guidelines; Work under the direction of a Case Manager/DON and collaborate with nurses, social workers, chaplains, volunteers, and STNA leadership; Support to deliver care 1-1 with patients and work independently as needed
