Role Description

This is a full-time, hybrid role for a Field Care Navigator located in San Francisco, CA. The Field Care Navigator will assist clients in accessing and coordinating care options as outlined in individualized Master Care Plans. Key responsibilities include conducting patient assessments, developing care plans, coordinating with healthcare providers and community resources, and providing ongoing guidance and advocacy for clients. The role requires frequent travel within the San Francisco area to meet clients in their homes or other settings to ensure comprehensive care delivery.


Qualifications

  • Experience in care coordination, case management, or social work in healthcare settings.
  • Strong communication, active listening, and interpersonal skills to effectively engage with clients and their families.
  • Organizational and multitasking abilities, including documentation and time management skills.
  • Proficiency in using health records and documentation systems for maintaining accurate client information.
  • Knowledge of community resources and healthcare systems, particularly the Medi-Cal program and managed care plans.
  • Ability to work collaboratively with multidisciplinary teams to implement comprehensive care plans.
  • Reliable transportation and flexibility for frequent travel to various client locations within the San Francisco area.
  • Bachelor's degree in Social Work, Nursing, Public Health, or a related field preferred; additional certifications or training in care coordination is a plus.
