Position Title: Social Worker

Client: CalOptima Health

Work Schedule: Monday – Friday, 8:00 AM – 5:00 PM

Salary Range: $32.70 – $40.00/hour

We are proud to partner with CalOptima Health, a respected public health plan serving Orange County, California, to fill a Social Worker position within their Multipurpose Senior Services Program (MSSP) — a program designed to improve the lives of older adults through coordinated care and compassionate support.

Position Summary

BuzzClan, on behalf of CalOptima Health, is seeking an experienced Social Worker to provide case management and care coordination services to elderly and high-need members through the MSSP Program. The Social Worker will collaborate with interdisciplinary care teams to assess member needs, develop care plans, connect members with community resources, and promote independence and quality of life for participants.

This is a full telework position requiring strong communication, organizational, and documentation skills, along with a commitment to CalOptima Health's mission of accessible, high-quality, and equitable care for all members.

Duties & Responsibilities

Care Management (70%)

  • Collaborate with an interdisciplinary care team to identify and address psychosocial needs, including financial issues, transportation, and community support services.
  • Coordinate the member's psychosocial care needs utilizing internal and external resources.
  • Work with utilization management and community partners to identify members who would benefit from case management services.
  • Conduct comprehensive assessments to determine member needs, strengths, and available resources.
  • Develop individualized care management service plans that incorporate assessment results, resource coordination, and education.
  • Serve as a liaison between CalOptima Health and community agencies, organizations, and state personnel.
  • Monitor all services provided to members, ensuring proper use of resources and timely delivery of care.
  • Evaluate member progress, anticipate future needs, and promote independence and decision-making.
  • Provide education and advocacy to members and families to enhance self-sufficiency and access to care.
  • Maintain detailed documentation of case management plans, interventions, and outcomes to demonstrate quality and cost-effective care.
  • Initiate case conferences as needed to address complex member needs.

Support Services (25%)

  • Participate in a mission-driven culture emphasizing quality, dignity, and accountability.
  • Support departmental goals by assisting with projects, initiatives, and process improvements.
  • Develop and maintain a network of community resources and support services for member referrals.
  • Collaborate with team members and leadership to achieve both short- and long-term program goals.

Other Duties (5%)

  • Complete special projects and tasks as assigned to support the department and organization.

Minimum Qualifications

  • Education: Master's degree in Social Work (required).
  • Experience:
  • Minimum 2 years of experience working with geriatric populations (required).
  • Experience in behavioral health, health facilities, health plans, counseling, case management, home health, or hospice (required).
  • Equivalent combinations of education and experience may be considered.
  • Licenses/Certifications:
  • Valid driver's license, reliable vehicle, and current auto insurance (for travel 50% or more of the time).
  • Technical Skills:
  • Proficiency in Microsoft Office Suite (Word, Outlook, Excel, PowerPoint).
  • Ability to use job-specific systems for documentation and reporting.

Preferred Qualifications

  • Bilingual proficiency in English and one of CalOptima Health's threshold languages (Arabic, Farsi, Chinese, Korean, Russian, Spanish, or Vietnamese).

Physical Requirements

  • Manual dexterity for computer use, writing, and paperwork.
  • Occasional lifting or moving of objects up to 25 lbs.

Work Environment

  • Full Telework: Primarily remote work environment with flexible scheduling; occasional travel may be required.
  • Community Work: May include occasional outdoor work and exposure to variable conditions when conducting member visits or field assignments.
  • Noise levels are typically moderate, and no harmful environmental conditions are expected in telework settings.
