Office Coordinator

Description:

The Office Coordinator plays a key role in organizing and supporting hospice care services. This position ensures accurate record-keeping, smooth communication among team members, and high-quality care for patients and families.


Contract 3-6 months

Pay range: $25-30/hour

Location: Santa Rosa, CA

Schedule: On-site Monday- Friday 8am-5pm


Key Responsibilities:

  • Enter and manage patient data, including referrals, admissions, and demographic updates.
  • Ensure all admission documents are complete and uploaded into the system.
  • Maintain patient records and generate compliance reports.
  • Track medical equipment and supply orders, reporting any issues.
  • Monitor physician orders and ensure timely processing.
  • Upload paper visit records into the EMR system.
  • Organize patient charts for easy access.
  • Confirm billing matches the ordered level of care.
  • Handle discharge paperwork and notify providers.
  • Request medical records and hospital charts as needed.
  • Coordinate with the interdisciplinary team for holistic care.
  • Serve as a point of contact for patients, families, and the hospice team.
  • Manage phone calls and ensure smooth communication.
  • Support the Director of Clinical Services with meeting prep and care plan updates.
  • Prepare facility binders with essential patient information.
  • Perform other duties as assigned.


Qualifications:

  • High school diploma or GED required.
  • At least 2 years of experience in medical records, office support, or a similar role preferred.


Skills:

  • Strong reading, writing, and communication abilities.
  • Ability to write reports and respond to inquiries professionally.
  • Comfortable using phones and interacting with patients and families.
  • High integrity and ability to maintain confidentiality.
