Job Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of skilled PT experience and required OASIS documentation skills.

Responsibilities
  • Home health setting covering Sonoma, CA and surrounding areas
  • Day shift, 5x8 hours per week from 8:00 to 16:30
  • Visit up to 6 patients per day with a standard of 30 productivity points
  • Weekend coverage required for one shift per contract
  • No on-call, no floating required between units
  • Travel pairs accepted but each must have their own territory and transportation
  • HomeCare HomeBase (HCHB) is used for all charting
  • No color requirement for attire
  • Core responsibilities include delivering physical therapy care at patient homes and documenting visits timely and accurately
  • Efforts are made to minimize drive time by grouping nearby clients but may occasionally require travel up to a 50+ mile radius
  • Free parking and mileage reimbursed at IRS standard rates
  • No standardized scrub color or attire required
  • No guarantee of block scheduling or alternative shift lengths
  • Strong and timely communication skills are mandatory
  • Each traveler will carry a separate patient caseload and territory
  • Immediate patient care and OASIS documentation experience are essential for this role
Requirements
  • Current BCLS certification required
  • Minimum of 1 year recent skilled physical therapy experience in a home health setting
  • OASIS documentation experience is required for completion of Start of Care
  • Proficiency in HomeCare HomeBase (HCHB) charting system required
  • Active driver's license and personal transportation required
  • RTO must be submitted at time of application
  • Applicants must not be first-time travelers
  • Completion of Home Health/Hospice checklist required prior to consideration
  • Pending license accepted for consideration, but license required if applicable
  • Applicants considered local if within 50-mile radius and may be accepted at the same rate
  • Former perm employees must be separated for at least 6 months and be HR approved
