Overview

Social Worker-Program Coordinator (CNH_ADHC) at U.S. Department of Veterans Affairs. Location: 2285 Challenger Way, Santa Rosa, CA 95407. Work Schedule: Full-Time, Monday - Friday; tour determined by supervisor. Remote/Virtual: Not authorized. This position is in the Excepted Service and does not confer competitive status.

Responsibilities
  • Provide clinical and programmatic oversight of Veterans receiving care in VA-contracted skilled nursing facilities and adult day health care settings.
  • Follow VHA directives and handbooks for CNH and ADHC programs.
  • Perform monthly oversight visits and coordinate annual facility inspections.
  • Address issues/complaints/concerns verbalized by Veterans and/or family members; advocate for high-quality care in placement settings.
  • Participate in quality improvement meetings and maintain partnerships with local ombudsman offices.
  • Serve as liaison between the Veteran and family, the community facility, and the SF Veterans Health Care System.
  • Act as an advanced practice clinician and subject matter expert on topics including star ratings, quality indicators, program eligibility, Veterans Care agreements (VCA), IDIQ contracts, SNF/ADHC waivers and approvals, and facility designation preferences.
Program Details
  • Program: Education Debt Reduction Program (EDRP) eligibility detailed by VHA Education Loan Repayment Services; program approval and award amounts (up to $200,000) and eligibility periods (one to five years) determined after review of the EDRP application. Former EDRP participants are ineligible to apply.
  • EDRP Authorized: See above; former participants ineligible to apply for incentive.
Requirements
  • You must be a U.S. Citizen to apply.
  • Selective Service registration required for males born after 12/31/1959.
  • Proficiency in written and spoken English.
  • May be required to serve a trial period.
  • Subject to background/security investigation.
  • Complete online onboarding; provide acceptable identification (I-9). REAL ID compliance will be required for certain forms of identification after May 7, 2025.
  • Must pass pre-employment physical examination.
  • Participation in seasonal influenza vaccination program is required for all VA Health Care Personnel.
Qualifications
Basic Requirements
  • United States Citizenship: Non-citizens may be appointed only when qualified citizens cannot be recruited per VA policy.
  • Education: Masters degree in social work from a CSWE-accredited program. Doctoral degree not substituted for MSW. Verification via CSWE.
  • Licensure: Must be licensed or certified at the advanced practice level in the state where employed; able to supervise licensure processes.
  • Physical Requirements: See VA Directive 5019.
  • English Language Proficiency: Proficient in English as required by law.
Grade Determinations
  • Social Worker (Program Coordinator) GS-12
    One year of experience at the GS-11 level demonstrating advanced practice skills and judgment; may include post-masters certification or internship experiences.
  • Licensure/Certification: Must be licensed/certified at the advanced practice level and capable of supervision for licensure.
Knowledge, Skills, and Abilities (KSAs)
  • Knowledge of program coordination, administration, and monitoring.
  • Ability to write policies, procedures, or practice guidelines.
  • Ability to supervise multidisciplinary staff.
  • Strong organizational skills, prioritization, and deadline management.
  • Ability to provide clinical training and orientation.
Additional Information
  • Travel: Up to 50% to various VA sites and partner facilities.
  • Driver's License: Active California drivers license required.
  • Education Verification: Only education from accredited institutions recognized by the U.S. Department of Education may be used for federal qualification; foreign education requires a Certificate of Foreign Equivalency.
