MANAGER REHABILITATION SERVICES - Providence Sonoma Home Health. Schedule: Full time Day shift. Manages and coordinates the day to day operations of the home health interdisciplinary rehab team to ensure quality and appropriateness of care, the availability of services, with appropriate staffing, facilitates and ensures quality initiatives and program implementation. Manages, maintains and sustains outcomes.
