Duties

• Provide physical therapy services to patients in their homes, ensuring personalized and effective care.
• Visit up to 6 patients per day, maintaining a 1:1 patient ratio.
• Complete 30 productivity points standard per day.
• Utilize EPIC/Oasis for charting and documentation.
• Work an 8-hour day shift, with one weekend shift required per contract.
• Travel within a 50-mile radius, with mileage reimbursed at the IRS standard rate.

• Basic Cardiac Life Support (BCLS) certification required.
• Experience with Oasis is preferred; please indicate if not experienced.

• minimum of 1 year of skilled Physical Therapy experience is required.
• Prior home health experience is not necessary, but first-time travelers are not accepted.
• Pending licenses will be accepted.
