The Coordinator of CARE Network is responsible for the daily operations and employee oversight of the CARE Network program. S/he assures federal, state, and local contracts and grants are managed and required reporting is accurate and timely. Assists with the formation and achievement of the strategies and goals for the CARE Network in alignment with the Community Benefit strategic plan. The CARE Network provides community based comprehensive case management services for the chronically ill, which may include CHF, Cancer, HIV/ AIDS and other chronic diseases.
