Outreach Case Manager

Costa Mesa, CA 92627

Salary: $23.24 to $28.28 per hour

Pay Frequency: 24 Pay Periods Per Year

Full Time, Non-Exempt

Driving Required

The lower end of range applies to candidates with minimum or limited relevant experience; the higher end applies to seasoned candidates with extensive relevant experience.

About Path

Since 1984, PATH (People Assisting The Homeless) has pioneered effective approaches to assist those experiencing homelessness. We operate services throughout California, including homelessness prevention, outreach, employment assistance, case management, supportive services, interim and permanent housing.

Job Responsibilities

  1. Conduct intake and needs assessments, develop Individualized Service Plans (ISP) addressing housing, income, health, and social goals.
  2. Provide referrals and support to help clients achieve their ISP goals.
  3. Complete program-specific assessments (e.g., VI-SPDAT).
  4. Evaluate client progress and conduct 90-day reviews.

Qualifications

Preferred Qualifications

Minimum Qualifications

  • All education and experience levels are welcome.

Mandatory Requirements

  • Employment eligibility verification.
  • Willingness to obtain CPR/First Aid certification.
  • Completion of TB test, background check, and drug screening.

Driving

Driving is essential. Candidates must have reliable transportation, a valid driver's license, proof of insurance, and the ability to qualify for PATH's insurance coverage.

Employment Eligibility

Applicants must be authorized to work in the U.S. without sponsorship. We do not employ individuals currently receiving services from PATH or who have received services in the past two years.

Work Location

All work must be performed within California. Remote work is not offered; hybrid schedules depend on organizational needs.
