POSITION: SLS Direct Support Professional

HOURS: Full-Time, Part-Time

PAY: $20–$26/hour

OVERVIEW

Optimum Care SLS II is a supported living services (SLS) vendor for the North Bay Regional Center serving adults with intellectual and developmental disabilities in Sonoma, Napa, and Solano Counties.

We are seeking qualified and experienced Direct Support Professionals to help us provide supported living services (SLS) to clients throughout Sonoma County.

You will work closely with an individual in their home and community, offer assistance with activities of daily living, promote independence, and ensure individuals' continued physical and psychological safety and well-being.

RESPONSIBILITIES

· Assist individual with ADLs, including personal care, meal preparation, shopping, and household chores.

· Support individual in developing and participating in community activities while successfully managing social, emotional, and behavioral factors.

· Implement individualized support plan and document progress and outcomes.

· Administer medications and provide basic healthcare support as needed, such as transportation to medical appointments.

· Ensure a safe, clean living environment for the individual.

· Advocate for the individual's needs and rights in community and other settings.

REQUIREMENTS

· Valid driver's license required.

· Vehicle with minimum insurance required.

· Clean driving record, as verified with a DMV records check.

· Able to pass DOJ/FBI LiveScan background check.

· CPR and First Aid Certificate(s) required.

· Negative TB test result from within the past year, or willingness to obtain.

· Completion of online provider trainings upon hire.

QUALIFICATIONS

· 1+ year experience in psychiatric care, nursing, or behavioral care highly preferred.
