Aging and Disability Resource Social Worker - County of Sonoma

The Sonoma County Aging and Disability Resource unit utilizes a no wrong door service delivery model, providing people of all ages, income levels, and abilities with equitable access to the information, assistance, and support they need to live as independently as possible in their own homes. This Aging and Disability Resource Social Worker will provide information and assistance, options counseling, short-term service coordination, and care management services to diverse, complex, and medically fragile individuals of all ages to support their needs in the community. Additional responsibilities include:

• Providing assessments of functional and psychosocial needs
• Responding to incoming calls and inquiries in a high-volume environment, identifying both expressed and underlying needs
• Providing timely and accurate resource information, referrals, and follow-up guidance to community members
• Applying strong clinical assessment skills to quickly evaluate situations and determine appropriate recommendations
• Maintaining comprehensive knowledge of community resources, aging and disability programs, and eligibility requirements
• Documenting all interactions and referrals in a client database
• Collaborating with internal programs, external partners, and medical staff to ensure seamless service delivery
• Providing person-centered services to older adults and to people with disabilities of all ages, their families, and caregivers

The ideal candidates will possess many of the following:

• Experience with social work case management and managing a complex caseload
• An understanding of Trauma-Informed Care and Diversity, Equity, and Inclusion (DEI)
• Significant knowledge of medical social work and mental health conditions for older adults
• Experience supporting individuals of all ages with disabilities, as well as older adults who are low-income, medically fragile, or have complex health needs
• Excellent interpersonal and communication skills, and the ability to interact with a diverse client population, with humility
• The ability to manage competing priorities in a fast-paced work environment
• Community-based care coordination, mental health, and counseling experience
• Experience conducting home visits with adults who are older and people with disabilities
• An interest and the ability to work within a multidisciplinary framework with physicians, nurses, and behavioral health clinicians
• A Master's Degree in social work, gerontology, marriage and family therapy, psychology, or a related field is highly desirable
