What you will do at AMC:
The Clinical Team Assistant (CTA) provides vital support to the nursing and care teams by assisting with non-clinical patient care coordination, documentation, communication, and workflow processes. Reporting to the Lead Nurse, the CTA plays a key role in ensuring efficient clinical operations and timely patient follow-up. This position supports smooth functioning of day-to-day activities and contributes to a high-quality, patient-centered experience.
* Assist with coordination and documentation of patient services including referrals, prior authorizations, and communication with external partners (e.g., specialists, pharmacies, labs).
* Manage refill requests including updating the pharmacy and provider information, ensuring appropriate routing and documentation.
* Assist with care gap reporting and follow-up (e.g., overdue labs, immunizations, mammograms), supporting panel management and quality initiatives.
* Communicate directly with patients as needed for scheduling, follow-up on test results, or health education materials, per clinical protocols.
* Support scanning, faxing, and uploading of medical records and documentation into the electronic health record.
* Help coordinate case management tasks under the guidance of the Lead Nurse, including patient outreach and support with Durable Medical Equipment (DME), IHSS coordination, Radiology Prior Authorizations or community services.
* Support panel reassignment processes and patient communication related to provider transitions.
* Monitor documentation workflows and follow up with team members to ensure timely completion of tasks.
* Participate in clinic meetings and continuous quality improvement (CQI) initiatives as assigned.
* Demonstrate a commitment to excellent communication, organization, and collaboration within the care team.
* Other duties as assigned.
Requirements
The Clinical Team Assistant at Alliance Medical Center has the following qualifications and skills:
* High school diploma or GED required; associate degree or post-secondary education preferred;
* At least one year of experience in a healthcare or clinic setting, required;
* Bilingual in English and Spanish required;
* Completion of medical assistant program with certification required;
* Current CPR certification required;
* Knowledge of medical office systems and terminology, including electronic health records;
* Strong organizational and communication skills;
* Ability to prioritize tasks and work effectively as part of a care team in a fast-paced environment;
* Proficient in Microsoft Office applications (Outlook, Word, Excel);
* Demonstrates cultural humility and professionalism in interactions with diverse populations;
* Ability to maintain patient confidentiality and adhere to HIPAA guidelines.
Work location: This position is expected to work onsite and not remotely.
